COVID-19 Office Policy

We are committed to taking every necessary step and precaution to ensure the safety of our patients and staff.

These are the steps we are taking to ensure the safety of our patients and staff.

All staff wear masks.
It is mandatory that you come to the office wearing a facial mask. Your mask must cover your mouth and nose and must be worn at all times (unless instructed otherwise by your provider) while inside of the building.
Office will be cleaned with CDC-approved disinfectants before and after each patient visit.
We screen each patient for COVID-19 symptoms and exposure prior to their appointment.
We check temperatures of all patients and employees upon entry into the office using a touch-less thermometer.
We practice social distancing throughout our office, maintaining six feet of distance, unless performing an exam or a procedure.
Patients must seen alone unless they are a minor (1 parent allowed) or need a caregiver during the exam.
We have hand sanitizers available throughout the office.
All necessary paperwork or forms should be completed online at least 1 day prior to your appointment to avoid your appointment being rescheduled. If you are unable to complete forms online, please call our office to make other arrangements.
Please arrive to your appointment 5-10 minutes early.
If you cannot make your appointment, we request a 24-hour notice to avoid being charged a no-show/late cancellation fee.
